Depending on your device and browser, these forms may be filled out electronically and emailed directly to the school office.

To Open and Complete Forms:

1. Download the form you need by clicking on its name. (Some browsers display a download arrow or box in the upper-right corner.)

2. Save the file to your device—such as your Downloads folder or another location you’ll remember.

3. Locate the saved form, then right-click and choose “Open with → Adobe Acrobat Reader.”

4. Fill out and save your completed form.

5. Email the saved form to the school office.
